Columbia St. Mary’s Physicians consists of over 200 physicians and 64 community clinics located throughout Milwaukee, Ozaukee, Sheboygan and Washington Counties. We are also associated with Madison Medical Affiliates', 12 clinics in southeastern Wisconsin. Our physicians provide care for the entire family and include many medical specialists. Birth Certificates
How
can I request a copy of my or my child's birth certificate?
Columbia St. Mary's cannot issue copies of birth certificates.
Copies of birth certificates can be requested from the County Clerk in
the county where you or your child was born or from the State of
Wisconsin Vital Records, Department of Health & Family Services.

Open enrollment is a once-a-year opportunity for you to re-evaluate your
current health care coverage and, if you wish, make changes. It’s
important to assess personal factors such as age, lifestyle and overall
health when selecting a physician. Columbia St. Mary’s has specialists
who can offer assistance in explaining the vast scope of programs and

PriceFinder allows patients to inquire about hospital
price information. To get your price estimate call PriceFinder at (414)
326-1900. PriceFinder services are available Monday through
Friday from 8:00 am to 6:00 pm.
Estimates provided by PriceFinder are an estimate of charges for the procedure/service without complications. This estimate does not include physician fees, such as radiologists, anesthesiologists, emergency room physician, or pathologists, or charges for any additional tests ordered for your care that are not routinely ordered as part of the procedure/service. Your final bill will include charges for the actual services provided to you. For questions about your financial obligation, we encourage you to contact your insurance company to verify details of your coverage.

& Pre-Register Outpatient Procedures
Columbia St. Mary's wants to make access to the services at our
hospitals as convenient as possible for you and your family.
When you call to schedule your hospital service, you can pre-register at the same time. Pre-registration may include asking for information such as your insurance carrier, physician name, address and more.
Patients who currently schedule appointments through a clinic or department other than Central Scheduling, should continue to do so. Please note that these departments are typically open during regular business hours, 8:00 am - 4:30 pm Monday through Friday.
