We recently transitioned our inpatient Hospice services from Columbia St. Mary's Hospital Columbia in Milwaukee to Columbia St. Mary's Hospital Ozaukee in Mequon. By partnering with Horizon Home Care & Hospice to manage these services, you can be assured that our care is supportive and compassionate, as well as focused on comfort and quality of life as patients near the end of life's journey.

We all desire the best for those we care about when they experience a terminal illness. Hospice is a special way of caring for people with life-limiting illnesses and offering support to their families. One of the goals of our Hospice is to help fill our patient’s days with comfort, compassion and dignity at a time when it matters most. Through hospice, our patient’s families are directly involved in making decisions and in caring for the person they love. We provide compassionate care and pain management as well as offer support for family members and friends.

Our new Hospice in Mequon provides a warm, spacious environment featuring 14 large all-private patient rooms, equipped with the comforts of home, in addition to comfortable sitting areas for family members. Our team of healthcare professionals is specialized in serving patients’ medical, emotional and spiritual needs.

For families seeking Hospice services on the east side of Milwaukee, Columbia St. Mary's and Horizon are developing partnerships with east side providers to enable us to provide Hospice care on the east side as well as in Mequon.
